Our Best Answer - How do I submit transcripts, recommendations, documents, and reports?

In addition to the college application, first-year applicants must ensure that the following required documents are submitted on their behalf. The following official forms can be found and submitted online through the Common Application (CA) or the Universal College Application (UCA), according to the instructions provided.

School Report and Transcript- Applicant's counselor or designated school official should submit the School Report Form (or the school’s own report form) and the student's official high school transcript.

Counselor Recommendation - Applicant's guidance counselor or designated school official should submit a written statement.

Teacher Evaluations - Two of the applicant's high school teachers should submit letters of recommendation. Ideally, teacher recommendations should be submitted by the teachers through the Common Application (CA), or the Universal College Application (UCA) website using the official forms and instructions provided. However, teacher recommenders may also mail their letters directly to Cornell's Application Processing Center at:

Teachers should include the applicant’s name and date of birth on all pages if sending letters by mail.

Mid-Year/Mid-Term Report – The applicant’s guidance counselor or designated school official should submit the Mid-Year/Mid-Term Report as soon after the application deadline as possible. High schools on a trimester system need not send a Mid-Year Report since the application deadline is not compatible with the trimester academic calendar.
